What is a Gynecomastia Surgery?

Gynecomastia surgery flattens and reshapes a male chest where breast tissue has developed. Most procedures combine liposuction to remove fatty tissue with direct removal of the firm glandular tissue behind the nipple, usually through a small incision at the edge of the areola. When there is significant loose skin, skin is removed as well, which adds scarring. It is a same-day procedure under general anesthesia or heavy sedation.

The key distinction is what the tissue actually is. Firm, rubbery tissue behind the nipple is glandular, and it will not respond to liposuction alone, diet, exercise, CoolSculpting or any fat treatment. Soft, fatty fullness sometimes can be treated with liposuction alone. A surgeon can usually tell the difference in an examination, and most patients have some of both.

One thing to take seriously before any cosmetic consultation: enlargement on one side only, rapid growth, a hard or fixed lump, skin dimpling or nipple discharge needs medical evaluation first, because male breast cancer exists. Gynecomastia can also be driven by medications, anabolic steroids or hormonal conditions, and an unaddressed cause can bring it back after surgery.

It depends on what the tissue actually is. If your chest fullness is soft and fatty, liposuction can treat it and non-invasive fat reduction may help. If there is firm, rubbery tissue behind the nipple, that is glandular and it will not respond to fat treatments of any kind. A surgeon can usually tell the difference during an examination, and most people turn out to have some of both.
Will it come back?
Once the gland is removed it does not grow back. What can change the result is a new cause. Significant weight gain, anabolic steroid use or a hormonal change can create new fullness. This is why surgeons want to know why it developed in the first place before operating.
