What is a Facial Fat Transfer?
Facial fat transfer uses your own fat as a filler. Fat is removed by gentle liposuction from a donor area such as the abdomen or thighs, processed, and injected in small amounts into areas of the face that have lost volume, most often the cheeks, temples, under-eye hollows and jawline. It is often done alongside a facelift or eyelid surgery.
Because it is your own tissue, the fat that survives is permanent. That is the appeal, and also the commitment: unlike hyaluronic acid filler, fat cannot be dissolved if you dislike the result. Filler is an off-the-shelf gel placed in minutes with no downtime that lasts six to eighteen months; fat transfer is a surgical procedure with real recovery and a permanent surviving result.
Expect the honest version of the timeline. The face looks overfilled at first, then a portion of the transferred fat is reabsorbed over three to six months, roughly half to two-thirds typically survives, and it varies by patient and by area. The final result is judged at six months, and a second session is not unusual.

Frequently asked questions
Why would I choose fat over filler?
Fat is your own tissue, so what survives is permanent and there is no ongoing cost every six to twelve months. It also suits larger areas of volume loss better than filler does. The trade-offs are real: it is surgery, there is a week or two of visible recovery, some of the fat is reabsorbed so the result is less predictable, and unlike hyaluronic acid filler it cannot be dissolved if you dislike it.
How much of the fat actually stays?
Not all of it. A portion is reabsorbed over the first three to six months, and the amount varies quite a lot between patients and between areas of the face. Surgeons plan for this by slightly overfilling at the time of surgery, which is why you look puffier at first than your final result. Some patients choose a second session once things settle.
Do I need to have enough fat to donate?
Almost everyone does. Only a small amount is needed for the face, and it can be taken from the abdomen, flanks or inner thighs. Very lean patients sometimes have limited donor options, and that is a conversation for the consultation.
