What is a Keloid Injections?
A keloid injection delivers a corticosteroid, most commonly triamcinolone (Kenalog), directly into raised scar tissue to soften and flatten it. The steroid slows the overproduction of collagen that made the scar grow beyond the original wound, and it usually calms the itching and tenderness before the scar visibly changes. It takes a few minutes in the office, and almost always works as a series of injections spaced four to six weeks apart rather than a single visit.
This is the standard first-line treatment for keloids, and that matters: cutting a keloid out on its own carries a high chance of it growing back, often larger, which is why surgery for keloids is nearly always combined with injections rather than used instead of them. Injections soften and flatten; they do not erase the scar, and keloids can recur, so maintenance is common.
It is not a treatment for indented acne scars, which are the opposite problem and belong with subcision, microneedling and resurfacing. And one honest note for the patients most affected: the steroid can lighten the skin around the injection site, and that shows more on deeper skin tones, the same skin most prone to keloids.
